On **(..SPECIFY DATE..) at **(..SPECIFY TOWNSHIP..) used on a road, namely **(..SPECIFY ROAD AND LOCATION..), a motor vehicle, namely **(..SPECIFY VEHICLE MAKE AND INDEX NUMBER..), fitted with a **(..SPECIFY LOCATION OF BELT..) disabled person’s seat belt in accordance with regulation 47 of the Road Vehicles (Construction and Use) Regulations 1986 when the ends of the belt were not, when the seat belt was being used for the purpose for which it was designed and constructed, securely fastened either to some part of the structure of the vehicle or to the seat which was being occupied by the person wearing the belt so that the body of such person would be restrained in the event of an accident to the vehicle

Contrary to regulation 48(4)(g) of the Road Vehicles (Construction and Use) Regulations 1986, section 42 of the Road Traffic Act 1988 and Schedule 2 to the Road Traffic Offenders Act 1988.
